The NextGen Bridge Program helps patients and families during vulnerable periods after a hospital stay, medication change, or change in condition. Nurses step in to provide short-term skilled support, teaching, monitoring, and stabilization-focused visits.
The program focuses on recovery, safety, and avoiding preventable complications while helping patients and caregivers feel more confident at home.

Services are tailored to immediate clinical needs after hospitalization, new medications, or changes in health. Final eligibility and visit frequency are confirmed case-by-case.
